Patient Portal FAQs

Using the Patient Portal

You can use the patient portal to:

  • View and request appointments
  • Send billing questions
  • Retrieve test results
  • View a summary of your medical record
  • Communicate with your Angel Family Health provider by sending and receiving secure messages
  • Update your personal information

Do not use the portal for medical emergencies, if you are having a medical emergency, please dial 911. If you are not feeling well and would like to speak with someone on your care team, please call (773) 782-2800

Any established adult patient may use the Patient Portal. Parents of Children ages 0-11 may use the patient portal on behalf of their child. An adult proxy may access an adult patient’s portal with the patient’s consent.

Adolescents (age 12-17) may receive their own account with a valid school or State I.D. Parents of non-emancipated adolescent patients will not be able to access their child’s medical information through the patient portal.

Parents of children ages 0-11 can have access to their child’s portal by requesting a personal identification number (PIN) for their child. This PIN will allow the parent to register their child and to connect their child’s information to their own account.

For access to a family member’s account, permission must be granted by the family member and a PIN will also be required in order to gain access to that person’s account.

Need to register for the Patient Portal?

To register for the patient portal, you must be an IWS patient with a valid e-mail address and a PIN number. You may request a PIN at the front desk before your next appointment. Once you register, you may change your PIN to a personal password.

Is my personal health information safe?

Yes, your personal health information is completely safe and secure. You must log in to view your personal health information, and you are the only person with the password to your account.

In order to request an appointment click on the “Scheduling” tab and then click on “Click here to schedule your appointment.” There you will be able to specify your provider, preferred appointment date, location and timeframe, and how you would like to be contacted with your appointment confirmation.

Please note that requesting an appointment does not guarantee that appointment time. Angel Harvey Family Health Center staff will follow up with you using your preferred method within 24 hours to confirm your appointment time.

You can find a summary of your medications, pharmacies, allergies, immunizations, lab results, and health conditions on the Patient Portal.
Please Note!

It is extremely important that you keep your pin or password in a secure location and to keep your pin or password completely confidential. Please DO NOT SHARE your pin or password with anyone. Your pin or password is specific to you and your secure account on the portal. It is your responsibility to keep your pin or password secure. Please change your pin or password if you feel that someone else may have access to your personal patient portal account.

Dr. Tara Ioffe

Family Practice

Dr. Tara Ioffe is a dedicated and compassionate Board-Certified Family Medicine physician with a strong commitment to providing excellent care to her patients. She obtained her medical degree from Avalon University in Curacao and went on to complete her Family Medicine Residency at the University of Oklahoma. She has worked at the University of Chicago and most recently NorthShore University. She has since returned to Chicago and is thrilled to join the IWS Family Health team in 2023, where she will continue to provide excellent care to her patients.

Throughout her career, Dr. Ioffe has gained experience in diverse medical fields, such as immediate and urgent care medicine while working at NorthShore Immediate Care. She did clinical research at the University of Chicago. Dr. Ioffe strives to foster a secure and inclusive environment for all her patients. She believes in an integrated approach to medicine, dedicating her time to understand each patient’s unique needs.

During her leisure time, Dr. Ioffe indulges in reading a variety of books for pleasure, plays boardgames, and loves to explore Chicago with her spouse and young child.
