Planned Giving: Create a legacy for future generations to enjoy

Join the Armour Heritage League with a gift to IWS Family Health through estate planning options.

The Armour Heritage League has been established by the Board of Directors to recognize and honor those people who have provided for IWS Family Health through their estate plans. Estate gifts are generally designated for an investment fund to benefit the IWS Family Health in perpetuity.

The continued growth of the investment fund is a key element in the fulfillment of our mission. The Armour Heritage League members truly recognize the future needs of the organization.

Generous donors may find that they can make a very significant contribution through the use of one of the several techniques available for estate planning.

Wills and Bequests . . . You can specify a dollar amount, specific assets, or specify a percentage of your estate and name the beneficiary of your living trust.  You may also want to name IWS Family Health a contingent beneficiary of a bequest intended for a surviving loved one, in the event the person does not survive you.

Gift Stock/Mutual Funds . . . Gifts of long-term appreciated stock and mutual fund shares are an easy way to make a lasting contribution to IWS Family Health. Benefits may include:

Income Tax Savings . . . For securities you have owned for more than one year, you can take an income tax deduction for the full market value, up to 30 percent of your adjusted gross income. The excess can be carried forward for up to 5 years.

Capital Gain Tax Savings . . . Avoid capital gain taxes you would have incurred if you had sold the stock or mutual fund shares.

Outright Gifts of Assets and Retained Life Estates . . . Gifts that may be given outright include long-term, appreciated stock, mutual fund shares, and real estate. Both cash and non-cash gifts offer certain tax advantages, while allowing IWS Family Health to use the gift immediately.  When real estate is given, the donor may retain a “life estate” and live on the property during his/her lifetime.

Many donors find giving appreciated assets to be a particularly attractive way to make a donation because they are supporting IWS Family Health at a lower cost than when making a cash gift.

Donor-Advised Fund

Grants from Donor-Advised Funds can be recommended to provide general operating support or to support a specific program. Please reach out to your donor-advised fund advisor to recommend a gift to IWS Family Health.

Commemorative Bricks: Donate a Brick & Pave the Way to Good Health

Over 200 bricks have been placed at the entrance of IWS Family Health to date.

We invite you to celebrate the memory of a loved one, commemorate a special occasion, or remember a friend—donate a commemorative brick to be placed at our entrance in West Logan Square. Your gift will help continue our tradition of visionary care and mark a special person or event in your life in a place where you will be proud to have helped open new doors to health care for those in need.

Therapeutic Books
Books are an important part of
the pediatric, dental, care coordination, CHAT, and counseling departments.
They are given to patients to reinforce the messages from our staff. Books can
be used to encourage behavior modifications, practice speech, alleviate anxiety
about visiting the clinic, and much more.

Cargo van/truck – IWS Family Health regularly picks up large equipment and donations. A cargo van/truck would save time and money on rentals.

We are regularly in need of exam room equipment, such as exam tables, instruments, and specialized testing machines. These requests change time to time based on the clinic needs and available funds.

Dr. Tara Ioffe

Family Practice

Dr. Tara Ioffe is a dedicated and compassionate Board-Certified Family Medicine physician with a strong commitment to providing excellent care to her patients. She obtained her medical degree from Avalon University in Curacao and went on to complete her Family Medicine Residency at the University of Oklahoma. She has worked at the University of Chicago and most recently NorthShore University. She has since returned to Chicago and is thrilled to join the IWS Family Health team in 2023, where she will continue to provide excellent care to her patients.

Throughout her career, Dr. Ioffe has gained experience in diverse medical fields, such as immediate and urgent care medicine while working at NorthShore Immediate Care. She did clinical research at the University of Chicago. Dr. Ioffe strives to foster a secure and inclusive environment for all her patients. She believes in an integrated approach to medicine, dedicating her time to understand each patient’s unique needs.

During her leisure time, Dr. Ioffe indulges in reading a variety of books for pleasure, plays boardgames, and loves to explore Chicago with her spouse and young child.
